ERIN SURRATT
MSW, LCSW
Erin Surratt is a Licensed Clinical Social Worker in Illinois. She received her Bachelor’s Degree in Psychology from the University of Illinois at Urbana in 2008 and then continued on at UofI for her Master’s Degree in Social Work, graduating in 2010 with a specialization in Evidence-Based Practices with Children and Adolescents. Erin has worked in multiple settings since 2006, including psychiatric inpatient units, a special needs preschool in Calgary, Alberta, community mental health, and outpatient behavioral health.
Erin has an extensive background in trauma and anxiety, specializing in work with children and adolescents. She is trained in EMDR and has completed multiple trainings on work with trauma, mood disorders, ADHD, and many more.

KAREN KEWLEY
LCPC, BCC
Karen Kewley’s education and trainings are extensive. Karen has a Masters in Counseling Education from Western Illinois University and my Board Certification in Life Coaching is from the Institute for Life Coach Training.
The therapies Karen uses most often are Solution Focused Therapy, Cognitive Behavior Therapy, Eye Movement Desensitization Reprocessing, and Mindfulness. Karen incorporates her Life Coaching skills with all the therapies she uses. Karen has experience working with children, teens, and adults including veterans. She draws from her 18 years of experience in mental health, 6 years of experience as a Life Coach, and 40 years of experience as an adult in helping you.

ANGELA ZUMBEK
MA, LCPC
Angela Zumbek is a Licensed Clinical Professional Counselor in Illinois. She completed her Bachelor’s Degree in Education from Butler University in 2004. After moving to the Peoria area, she completed her Master’s Degree in Counseling from Bradley University in 2009. Angela completed the required coursework to become licensed both as a school counselor as well as a clinical counselor. Angela has worked in the mental health field since 2005 with children, adults, and families in a variety of settings.
Angela has a background in working with trauma, attention deficits, anxiety, and various mood disorders. Angela has engaged in many training throughout her career. This includes extensive training related to Dialectic Behavior Therapy, Cognitive Behavior Therapy, and more. Angela is open to working with adolescents, adults, and families in order to help you to feel success and balance in your life.

People seek counseling for a variety of reasons: to address difficult thoughts, painful emotions, grief and loss, relationship conflicts, parenting issues, gender identity, sexuality, stress, anger, compromised self-esteem, low motivation, trauma, habits, addictions, and more. Sometimes we seek counsel because we are ready to make changes in our lives and we want support and feedback for the journey. Finding a good counselor can make all the difference. My specialty is mental health. I have worked with a broad spectrum of clients addressing a variety of issues including anxiety, depression, serious mental illness, stress, perfectionism, grief, anger management, aging, trauma, gender identity, and more.
It is my goal to work together to help you identify goals and achieve those goals.
Stephanie Dunn
MS, QMHP
Stephanie is our newest addition currently working part time with us. Stephanie graduated from Eastern Illinois University for both her Bachelors and Masters in Counseling and Family Services. Stephanie has previously worked as a Therapeutic Residential Counselor and Foster Care Case Manager. Stephanie has been a therapist working with families in within the foster care system since 2007. Stephanies specialties and trainings include grief and loss, DBT, anger and stress management, therapeutic crisis intervention, trauma focused CBT, motivational interviewing, and ARC trauma treatment. Stephanie has facilitated parenting classes, DV classes, aa well as teen therapy groups to work on developing social skills. Stephanie is a wonderful addition to the team with years of experience.
Mariah DeCrane
MBA, MHA
Mariah is our Jane of all trades, or our Chief Administrative Officer. Mariah found her passion for working in mental health while earning her undergraduate degree from the University of Southern Indiana in 2014. Mariah began her career working in developmental disabilities with Easter Seals and expanded to outpatient behavioral health, then expanded further working in emergency response services handling Crisis Center calls in the greater Peoria area. Mariah completed her Masters Degrees in 2017 while working for two large Peoria-area hospitals in various administrative aspects. Fast forward a few years to when she took a full-time position here at Phoenix Wellness, PLLC. Mariah is our go-to girl for anything you need, she's got you.

2012-2023
Our resident golden doodle, born in 2012, belonged to Erin and was often found on a couch sleeping or in the waiting room getting pets and treats. He started coming to work when he was diagnosed with an autoimmune disorder in 2021, so he could be monitored. Then every morning he would make sure he got to come to work! He loved greeting people in the waiting room with his roo-roos and a toy. In his spare time he loved naps, playing with his toys, and giving kisses. Barkley will forever be missed!
Maci Zumbek
Executive Happiness Coordinator
Maci is a facility service dog. She trained with Paws Giving Independence in Peoria, Illinois. Maci remains up to date with training requirements and annual public testing through PGI. For more information about PGI, please go to www.givingindependence.org.
Maci loves coming to work to help people to feel more comfortable and at ease as they work through the various challenges that arise. She gets a bit excited when meeting new people and may talk to you as you arrive. She also loves to show off her toys to anyone she meets.
In her free time she loves to play ball, take naps, and play with your brother Rex.
Winston Surratt
Therapeutic Nuisance
Winston was born October 19, 2023 and came home to his forever home with Erin and her family on December 14, 2023. He is working on training and socialization in order to help him grow into a well rounded doggo and therapy dog. He has passed puppy class and beginner obedience training and is in the process of working on his intermediate obedience training and his AKC Canine Good Citizen. He comes to work most days with Erin at the office. Winston loves coming to work, playing with toys, chewing bones and naps.
Rex Zumbek
Lead Relationship Specialist
Rex was adopted in May 2021 as a young pup. He is working on training and socialization in order to help him grow into a well rounded doggo. Sometimes he is able to come to work to share his joy. Rex loves meeting new people and puppies. He will likely greet you with a whole body wiggle and a smile.
